Access to Healthcare: Transport Needs Survey

PACTO is undertaking a community consultation to help shape the future Pathway to Home & Health programme, and they would be grateful for your input. PACTO would like to hear from you if you live in Pembrokeshire and you.....

• have experienced difficulty accessing healthcare because of transport;

• have recently returned home following a hospital stay or treatment;

• have recovered at home following illness;

• have experienced a fall or reduction in mobility;

• have needed short-term support to remain independent;

• are disabled or living with a long-term health condition;

• are unpaid carers;

• live alone or in a rural community;

• do not drive or have regular access to a car; or

• simply have views about what would help people remain independent and connected.

Why does this matter?

PACTO want to hear directly from local people about what actually happens when transport, ill health or reduced mobility makes everyday life more difficult, what support works, what is missing and what they would change.  Feedback will be used to change and shape the service itself.

The consultation should take approximately five minutes, and people will be able to participate online, by telephone, on paper or through a PACTO-facilitated conversation.
